Symptom Checklist

Whether you think you may be suffering from depression or are currently being treated, the symptom checklist allows you to learn about depression and the symptoms you may be experiencing. It also allows you to rate how much these symptoms impact your life. Choose a number from 1 – 10 where 10 indicates the highest impact to you. It is important to remember this is not a “score” but a way to communicate how much you believe the symptom affects your day-to-day life.
