It also allows you to rate how much these symptoms impact your life. Choose a number from 1 – 10 where 10 indicates the highest impact to you. It is important to remember this is not a “score” but a way to communicate how much you believe the symptom affects your day-to-day life. When you finish, you will be able to print out your symptoms and share this information with your doctor.
This is not a diagnostic tool. In other words, completing this checklist will not tell you whether or not you suffer from depression. The purpose of this tool is to help you discuss your symptoms with your doctor.
